FROM THE NORTH
All East Georgia Communities, S. Carolina, Alabama and North Florida or Jacksonville
Orange Park, Palatka, Daytona, St. Augustine, Palm Coast, Orlando and all areas south of Jacksonville
Tallahassee, Pensacola, West Georgia (Albany area), Panama City, Gainesville, Starke, and all other areas to the West of Jacksonville and North of Tallahassee
FROM THE NORTH
All East Georgia Communities, S. Carolina, Alabama and North Florida or Jacksonville
- I-95 South to Jacksonville. If you are coming from west Georgia, it is best to take I-10 to I-95 south in Jacksonville. South on I-75 is a good way to get to I-10 east. Once you have reached I-95 south, stay on this interstate and pass by downtown Jacksonville on your left. If coming from the North, stay in the left lane of I-95 to continue south. There will be a sign which indicates a left pathway to stay on I-95 south. This will take you over the river. You will get off I-95 to the right on EXIT 346A (University Blvd. - East) which is the second University Blvd exit. There are two University Blvd exits and you be taking the second one. Exit the ramp and you will now be heading in the correct direction on University Blvd.
Once you have exited I-95 and have reached University Blvd, these are the directions which will lead you directly to the facility.
- Go 1.5 miles to 3599 University Boulevard S. You will pass a variety of shops and strip mall plazas. On your right you will see a very large facility which is Memorial Hospital. There are three entrances to the hospital and you need to go to the third entrance, which will have a blue sign showing the "Emergency Entrance/Samuel Wells Medical Complex."
Our facility is not able to be seen from the street, as it is located directly behind the Memorial Hospital in a private medical complex, which is accessed by turning into the emergency entrance to the hospital. For your privacy, there are no signs visible from University Blvd representing our facility.
- Turn right at this hospital entrance which also has a green street sign which indicates Samuel Wells Drive. It will seem as though you are turning into the hospital due to our location.
- After you turn right, follow the road into the complex which will take you under a pedestrian overpass walkway between Brooks Rehab Center (on your left) and the emergency entrance to Memorial Hospital (on your right). Follow this road which will lead you to a 4 way stop sign.
- At the stop sign, go straight and you have entered the Samuel Wells Medical Complex. You will have a slight bend to the right and then you will drive through to the second building from the end on the left. The numbers of the building will increase as you drive further into the complex. Our facility is clearly marked with our name on the front and a large 1200 on the side.

- I-95 North to Jacksonville. Exit 345 (Bowden Road to University Blvd.) At the bottom of the ramp turn right, then at the first light (Spring Park) turn left. Spring Park road will take you to University Blvd.
- Turn right onto University Blvd.
Once you have exited I-95 and have reached University Blvd, these are the directions which will lead you directly to the facility.
